Laboratory Tests Ordered by Phone

Phone orders from authorized individuals are accepted; however, the Clinical Laboratory Improvement Act of 1988 requires testing laboratories to obtain written follow-up confirmation of all tests ordered. No report will be issued without a completed laboratory requisition form.

Cancellation of Tests

Request for cancellation received prior to test setup will be honored at no charge. Requests received following test setup cannot be honored. A report will be issued and charged accordingly.
